Medical Forms

A job at sea is physically demanding. The marine industry and the college requires proof that you are capable of meeting these demands by having a doctor perform a medical examination.

Applicants taking MEDs A1, A2, STCW Basic Safety, STCW Proficiency in Survival Craft, STCW Advanced Fire Fighting at the Port Colborne MED Training Centre must be physically fit.

Training in marine fire fighting and in the use of lifeboats and life rafts requires a participant to engage in moderately strenuous physical activity.

Marine Training activities include:
• Fighting fires while wearing heavy fire protective clothing and breathing Apparatus
• Working in areas with little or no visibility
• Righting an inverted life raft
• Treading water and swimming in water up to 10 feet in depth.

A Marine Medical Fitness Report is mandatory and must be submitted to Dianne at the MED Centre, at the beginning of the course on Day 1 or you will be denied entry into the class.

There are two types of Marine Medical Fitness Reports:

1. A Transport Canada Marine Medical Report is completed by a Transport Canada Approved Doctor. This type of medical is required by most shipping companies/Coast Guard etc. A CDN number is required on this certificate or it will not be processed by Transport Canada. This form can only be obtained at the doctor’s office. To check for a Transport Canada approved Doctor check here: http://wwwapps.tc.gc.ca/saf-sec-sur/4/mme-mdm/m.aspx?lang=e

or

2. The Georgian College Marine Medical Report form can be completed by your family physician/ health clinic etc. This form can be found here: http://marinetraining.ca/glimtc/wp-content/uploads/mfr.pdf

Please note: The MED Centre requires only one medical form, either the Transport Canada Marine Medical Form or the Georgian College Marine Medical Form, not both, but you must bring one of them to class on Day 1.

The MED Centre will accept any proof of a medical, including any past medicals, Transport Canada Aviation medicals; Health Canada and Commercial Divers medicals provided they are no more than one year old.

Attendees will be required to sign a Consent and Release Form attesting that they will personally assume any and all risk associated with this program.
